To schedule an appointment, call KC Endodontics. Dental offices can submit referrals through the Jotform embedded on our referral form page. Information submitted through that form is processed by Jotform for KC Endodontics. A fillable PDF is also available for offices that prefer to print a referral.
Do not send protected health information through ordinary, unsecured email. Dental offices should call KC Endodontics to confirm the practice’s current secure method for sending referrals, diagnostic images, or clinical records.
Standard hosting systems may process limited technical information needed to deliver and protect the website, such as an internet protocol address, browser type, requested page, referring page, and date or time of access.
