What to send with a referral
When available, please include:
- The tooth or area to be evaluated
- The reason for referral and relevant symptoms
- Recent diagnostic images
- Pertinent treatment history and restorative considerations
- Current medications or medical considerations that may affect care
- Your preferred scope of treatment and follow-up communication
Please do not send private patient information through ordinary, unsecured email. Call the office at 913-642-3636 if you need help choosing a secure delivery method.
Communication and restorative follow-up
Diagnostic findings and completed care are communicated to the referring office. When a final restoration or other follow-up is needed, the patient is directed back to the restorative dentist with clear timing guidance.
For an urgent referral or a case you would like to discuss directly, call the office. The team will help coordinate the most appropriate next step.
